Earlier this year, KishHealth Physician Group opened a new location to see patients at Fox Valley Older Adults Center, 1406 Suydam Road, Sandwich.
On a rotating basis, physicians from other KishHealth Physician Group locations in Sandwich and Plano will see patients at FVOAS from 9 a.m. to 1 p.m. Thursdays. The clinic at FVOAS is similar to other KPG locations, with a nurse on staff and a pleasant waiting area and clinical space.
The partnership with FVOAS is part of KishHealth System’s mission, and a result of a communitywide needs assessment that was conducted in 2012.
The needs assessment identified a wide variety of priorities, including expanding access to care and older adult health. The new clinic on site at FVOAS creates more access by placing a physician and other clinical staff in a comfortable and known setting, and allows the health system to directly impact the health of local older adults.
Three local KPG family practice physicians, Drs. Jamie Broekhuizen, Maureen Dela Cruz and Syeda Ali, will rotate out to FVOAS and provide needed services.
“We’re in the midst of a fundamental change in the way health care is being delivered,” Kevin Poorten, KishHealth System CEO, said in a news release. “The health system, through our physician group, is working to be a leader in that change by moving care outside of the hospital, and helping the entire community manage its health.”
In addition to the physician clinic, the health system Community Wellness and Physical Therapy Departments are providing fitness and other wellness classes on site at the community center.
KPG became part of KishHealth System in 2011 starting with Drs. Erik Englehart and Azeem Haleem and employs primary care and specialist physicians throughout the DeKalb and Sandwich areas.
“We really feel that patients who choose one of our health system physicians benefit from greater access to care,” Abe Chacko, executive director of KishHealth System Physician Group, said in the release. “We accept a broad base of insurance plans, including Medicare and Medicaid, and have a fee scale for those without insurance. And through the support of the health system, our physicians have the latest technology, equipment and education to provide patients a really high level of care.”
KishHealth System and FVOAS will celebrate their partnership and the opening of the clinic with a public open house from 5 to 7 p.m. Nov. 14. The open house features activities and a blood pressure screening, and the Sandwich Area Chamber of Commerce will host an official ribbon-cutting at 5:30 p.m. Full results of the community needs assessment, and KishHealth System’s strategy and implementation plan can be found on the health system website.
