SYCAMORE – Voters who are permanently physically incapacitated and unable to make it to a designated polling place on election day may qualify to automatically receive an application for an absentee ballot before every election for the next five years, according to the DeKalb County Clerk’s Office.
People who believe they qualify for this service are asked to visit the clerk’s website at www.dekalbclerk.com and download the “Disabled Voter Application Form.”
This form must be finished and returned with a sworn statement from a physician indicating a person is eligible for the service, or provide a disability card issued by the state for verification, according to a news release from the clerk’s office.
Voters may also call the clerk’s office at 815-895-7147 or visit it at 110 E. Sycamore St. in Sycamore to sign up for the program.
